To cure a disease in The Elder Scrolls IV: Oblivion Remastered, drink any Cure Disease potion, cast a Cure Disease spell, eat a Cure Disease ingredient such as Mandrake Root, or pray at the central altar of any city chapel. All of them clear every disease you are carrying at once, and none needs a specific cure per illness. This guide covers how you get infected and how to check, each cure in order of how quickly you can reach it, the chapel altar’s one condition, and the three-day rule that makes Porphyric Hemophilia different from everything else.
How you catch a disease and how to tell
Diseases come from the melee attacks of specific creatures. Per the community documentation on UESP, rats, wolves, bears, mountain lions, boars, deer, dogs, sheep, mudcrabs and slaughterfish each carry one or two, zombies and headless zombies carry many, and beggars carry them too but only pass them on if you attack them. Characters with Resist Disease are infected less often, with two exceptions that ignore resistance entirely: Astral Vapors and Porphyric Hemophilia.
Infection is announced by an on-screen message naming the disease. After that it lives in your active effects, which the remaster shows in the lower left of the HUD as well as in the journal’s magic section. Most diseases drain one to three attributes by 5 to 10 points each; Collywobbles, for example, drains Strength, Endurance and Speed by 10 apiece, and Helljoint drains Agility and Speed by 5. Multiple diseases stack, so a long dungeon can leave you noticeably weaker without a single dramatic symptom. Update 1.1 fixed a launch bug where a character’s skin textures stopped matching after catching a disease, which was the one visible tell some players relied on.
Cure 1: potions and scrolls
A Cure Disease potion is the fastest fix and the one to carry. They turn up for sale at alchemists and chapels and in dungeon loot, and you can brew your own with any two ingredients that share the Cure Disease effect. The effect has no magnitude or duration, so a weak homemade potion works exactly as well as a bought one. Some scrolls carry the same effect.
Cure 2: eat an ingredient
You do not need to brew anything. Eating a raw ingredient gives you its first alchemical effect, and four common ones list Cure Disease first, per UESP’s ingredient tables: Mandrake Root, Clannfear Claws, Root Pulp and Shepherd’s Pie. Mandrake grows wild across the western Colovian Highlands, the Gold Coast and near Bravil, and The Main Ingredient in the Imperial City Market District stocks the root; Shepherd’s Pie is a food item found in some homes and kitchens. Elf Cup Cap also cures disease, but only at Alchemy 25 or higher because it is the mushroom’s second effect. The 2025 version of this guide listed Redwort Flower as a cure; that flower cures poison, not disease.
Cure 3: cast a spell
Cure Disease is a Restoration effect, and vendor spells carrying it are sold by chapel priests and Mages Guild Restoration merchants. The spell’s exact rank varies by which version you buy, so check the rank shown in the Magic menu against your Restoration skill. Once you have a Cure Disease spell you can also build your own at the Arcane University’s Altar of Spellmaking, since the effect is only available to spellmaking through a known spell. Our spells guide covers the cast buttons and skill ranks on each platform.
Cure 4: the chapel altar, and its one condition
Every major city has a chapel with ten altars. The central one cures all diseases, restores every attribute by 100 points and restores 200 health, for free, as often as you like. Walk up and press interact (E on keyboard, A on Xbox, X on PlayStation).
There is one condition, and the 2025 version of this guide got it wrong. Per UESP, the central altar refuses you when your Infamy is higher than your Fame, or when you have an active bounty. Dark Brotherhood membership by itself does not lock you out; it is the Infamy those quests add that eventually tips the balance. If the altar refuses you, pay off the bounty first. Wilderness wayshrines give the same restorative blessing, but the remaster includes Knights of the Nine, and with that content installed wayshrines only work at zero Infamy, so they are stricter, not looser. Knights of the Nine also adds the Pilgrimage quest, which clears your Infamy by visiting one wayshrine for each Divine. If your Infamy is high, a potion or an ingredient is simply the easier route, and the Fame you earn from ordinary quests slowly reopens the altar. Our guide to raising disposition covers the reputation system’s other side.
Porphyric Hemophilia: the three-day rule
Porphyric Hemophilia, caught from vampire melee attacks, is the one disease with a clock. For 72 in-game hours it is an ordinary disease that any of the cures above removes, and it shows as a 5-point Drain Fatigue. After three days, Cure Disease stops working, and the next time you sleep you begin the change into a vampire. If you were bitten in a vampire lair and do not want to turn, cure it before you rest. If you do want to turn, our guide to becoming a vampire explains what happens next, and how to cure vampirism covers the long quest that is your only way back afterwards. The rest of our coverage is on the Elder Scrolls hub.
Frequently Asked Questions
What is the fastest way to cure a disease in Oblivion Remastered? Eat a Mandrake Root or a Shepherd’s Pie from your inventory. Both cure at any Alchemy level, and both are common.
Does one Cure Disease remove all my diseases? Yes. The effect clears every disease at once, however many you are carrying.
Why does the chapel altar not work for me? Your Infamy is higher than your Fame, or you have a bounty. Clear the bounty, and until your Fame catches up, eat a Mandrake Root or drink a potion instead.
Can vampires catch diseases? Full vampires have 100% Resist Disease. On the remaster, per UESP, they can still contract Porphyric Hemophilia itself, which has no further effect on someone already turned.
Can my horse catch a disease? Per UESP, yes, and a horse that stops healing may be sick. A wayshrine blessing applied to the horse cures it; see our horse guide for how horses work in the remaster.
