The IV bag in Silent Hill: Townfall is a one-time extra life: with a filled IV bag equipped, Simon is revived with a small amount of health the first time he would otherwise die. To use it, you combine an Empty IV Bag with IV Fluid in your inventory, then equip the filled bag.
This guide explains how the revive works, how to fill and equip the bag, where early players found the first empty bags and fluid, and how to fit it into your healing strategy. It covers the launch version on PS5 and PC (Steam and Epic), released September 24, 2026.
What the IV Bag Does
The IV bag is Townfall’s safety net. Launch reviews describe it in the same terms:
- Noisy Pixel wrote that while Simon “has a full IV bag equipped, the first time he would otherwise be knocked unconscious, you get a second wind with some health restored rather than hitting the game over screen.”
- Gaming Trend called it an IV bag “that acts as a second life if he is fatally struck.”
- GameGrin noted that the resource containers you open “will usually contain some healing items, especially spare IV bags and fluid that will bring you back if you die.”
A few rules follow from that:
- It only works when equipped and full. An empty bag, or a filled bag sitting in your inventory unequipped, does nothing.
- It triggers once. After it revives you, it is spent. You need another fluid refill before it will save you again.
- It restores only a little health. The revive keeps you alive, not safe. Get out of danger and heal properly straight away.
- It is Simon’s. Noisy Pixel describes IV bags as “unique to Simon,” so do not count on one in sections where you control another character.
It fits the game’s setting, too. Konami’s PlayStation Store listing notes that Simon arrives in St. Amelia with nothing but his clothes, a medical wristband and an IV bag. Our spoiler-free story guide covers that opening.
How to Fill and Equip the IV Bag
Early walkthroughs describe the same three steps:
- Find an Empty IV Bag and some IV Fluid. They are separate pickups.
- Combine them in your inventory. Open the inventory, select one item and combine it with the other, which puts the fluid inside the bag.
- Equip the filled bag. Once it is equipped, the revive is active.
Combining is the same system Townfall uses for crafting better healing items, and the trophy list rewards it: “From Scratch” asks you to create 20 new items by combining others. It pays to combine as you go rather than hoarding raw parts. Our trophy guide lists the other crafting and resource trophies.
Where to Find Empty IV Bags and IV Fluid
Townfall does not have one fixed IV bag location. Parts turn up in medical areas and in the resource containers scattered around town. Early walkthroughs from launch week agree on two early sources in Part 1:
- The clinic. Walkthroughs describe finding IV Fluid in a locker in the clinic, in the room labeled for B. McMillan.
- Building 16. A lock box, opened with a Lock Box Key found nearby, reportedly holds both an Empty IV Bag and IV Fluid.
After that, keep opening every resource container you pass. GameGrin’s review says they “usually” contain healing items, IV parts especially. Opening containers also feeds the “Resourceful” trophy, according to trophy guides.
If you are struggling to remember which rooms you have cleared, Simon marks searched areas on the map. Our map guide explains the markings.
The IV Bag and Your Other Healing Items
The IV bag is not how you heal day to day. For that, reviewers and walkthroughs describe:
- Gauze, the most common healing item.
- First Aid Kits, which walkthroughs report finding in places like the pharmacy.
- Crafted upgrades. Noisy Pixel describes combining healing items with antibiotics or alcohol to make them more potent, and GameGrin gives improving gauze as an example.
One caution from reviewers applies to all of them: healing plays an animation that leaves you open, so heal after breaking line of sight, not mid-swing. Our combat and weapons guide covers melee, blocking and the rest of the survival toolkit.
When to Keep One Ready
Think of the IV bag as insurance for the moments you cannot plan around:
- Stalker sections. Reviewers describe an unkillable pursuer that kills on contact. A filled, equipped IV bag gives you one mistake. Our bosses and monsters guide has survival tips for those stretches.
- Higher difficulties. On harder combat settings, every mistake costs more. Our difficulty guide explains each option and how to change it mid-run.
- Long gaps between phones. Manual saves use telephones, so a revive can spare you a long replay. Our saving guide explains how saving works, including the trophy that forbids manual saves after a certain point; on that run, an IV bag matters even more.
The simple habit: the moment a revive is used, refill the bag as soon as you have fluid. For more Townfall help, see our Silent Hill guides hub.
Frequently Asked Questions
What does the IV bag do in Silent Hill Townfall? With a filled IV bag equipped, Simon is revived with some health the first time he would otherwise die. It is used up when it triggers.
How do you fill the IV bag? Combine an Empty IV Bag with IV Fluid from the inventory, then equip the filled bag.
Where do you find IV Fluid? Early walkthroughs point to a locker in the clinic and a lock box in Building 16, and reviewers say resource containers often hold IV parts.
Does the IV bag heal you? Only a little, and only when it revives you. Use gauze and first aid kits for regular healing.
Is the IV bag the same as a save? No. It keeps you alive on the spot once, while saves happen at telephones. Use both: save when you can, and keep a filled bag equipped between phones.
